If deciding to work with me, it can be helpful if you review some paperwork before our first in-person meeting.

These documents may include:

• Informed Consent Form
• Initial Client Information Form
• Notice of Privacy Practices & Confidentiality Policy
• Private Practice Social Media Policy
• Consultation Form

If possible, bring copies of your completed forms to your first appointment. If you are unable to do so for any reason, I am happy to provide you with hard copies of these forms when we meet.
